Just got this today.

Your stream has been added to the teamliquid stream list.

Once you turn on your stream you will be listed on http://www.teamliquid.net/video/streams/. You can edit your stream settings (race, description, language, and online/offline status) through the 'stream' link at the top left - http://www.teamliquid.net/mytlnet/mystream.php. If you're streaming SC2 make sure you set the game to Starcraft 2 to get the [SC2] tag in the sidebar. If you need to change your stream channel/site or thread, you will need to submit a new application.

For quick access, there is an arrow next to your stream link at the top left of every TL page. You can click the arrow to quickly toggle your stream on and off. Please remember to set your stream as offline when you are done streaming, repeatedly leaving your stream running on TL when it is offline may result in it being removed.
